Effective immediately when there has been no administration of a decedent’s estate, the following forms may be used to establish an individual’s authority to act for, bind, and accept payment on behalf of a decedent/injured party and his/her estate and heirs for claims filed with the CPF.
Law firms and claimants may submit one form or the other with respect to a particular claim; both forms need not be completed.
