  1. MESOTHELIOMA
    • 10 year latency
    • Pathology report

  2. LUNG CANCER
    • 10 year latency

      1. Medical report by a physician diagnosing primary asbestos-related cancer of the lung, confirming knowledge of claimant's work history and/or exposure to asbestos and that said injury was caused or contributed to by that asbestos exposure.

      2. OR

      3. Diagnosis by X-ray of lung cancer and an underlying asbestos injury along with clinical findings supporting the diagnosis, confirming knowledge of claimant's work history and/or exposure to asbestos and that said injury was caused or contributed to by that asbestos exposure.

  3. OTHER CANCER
    • 10 year latency
    • Medical report diagnosing primary asbestos-related cancer.
    • Diagnosis by X-ray of underlying asbestos disease or injury, confirming knowledge of claimant's work history and/or exposure to asbestos and that said injury was caused by or contributed to by that asbestos exposure.
    • If no X-ray is available, the medical report must confirm knowledge of claimant's work history and/or exposure to asbestos and that said injury was caused or contributed to by that asbestos exposure.

  4. NON-MALIGNANCY
    • 10 year latency
    • Findings by X-ray, CAT scan, autopsy, etc. of asbestos injury.
    • Medical report by a physician outlining the condition, symptoms and work history and/or exposure to asbestos of the claimant.
    • Diagnosis of an asbestos disease or injury based on B. and C. (above) referencing any materials reviewed.
    • If deceased and C. (above) is not available: an X-ray diagnosis (B. above) or pathology with separate diagnosis, in either case confirming knowledge of claimant's work history and/or exposure to asbestos and that said injury was caused or contributed to by that asbestos exposure. A death certificate is of course mandatory in all such cases.


  5. FOR OPTION 2 CLAIMS, THE FOLLOWING ADDITIONAL INFORMATION IS NECESSARY TO REALIZE FULL VALUE.
    • PFT's
    • Full physical examination.
    • All medical records or information deemed appropriate.
