Efile Agreement (EFA)

In order to access the Efile system, you will need to register with the Claims Processing Facility, Inc. (CPF). This can be done by downloading and completing the EFA. You must return a signed copy of the EFA to the CPF either through the mail, by fax or by e-mail. Failure to complete the entire form may result in processing delays. Your EFA will be reviewed and when approved, the CPF will create an account for your law firm and you will be notified.
