All claimants suffering from an asbestos injury resulting from exposure to UNR asbestos-containing products must complete the standardized UNR form in its entirety and select either Option 2 (Individualized Resolution) or Option 3 (Deferral). Option 2 is an in-depth individualized review of the claim using basic tort system principles to review the claim. Payment of valid Option 2 claims requires the execution of a full release for either the one-payment plan (70% of the liquidation value) or the two payment plan, paid over two years. Option 3 allows a claimant to defer consideration of the claim for up to a maximum of two years from the date of filing. All claims must be accompanied by a $100.00 non-refundable filing fee. Checks should be made payable to the UNR Asbestos-Disease Claims Trust.

To file a claim, please follow these steps:

1. Complete the standardized UNR claim form in its entirety and sign the certification of accuracy and completeness at the end of the form.
   
   
2. Attach the required medical documentation supporting the alleged injury.
   
   
3. Enclose the $100.00 non-refundable filing fee.
   
   
4. Return the completed form and required documentation along with the filing fee to the CPF.
